Initial Assessment for Nurse Practitioners: $250 + $25 UDS
Yearly Evaluations for Nurse Practitioners: $200 + $25 UDS
Follow-up fees for Nurse Practitioners are as follows:
- Every 30 days: $100
- Every 45 days: $130
- Every 60 days: $165
- Every 90 days: $215
Initial Assessment for Therapists: $175
Follow-ups for Therapists: $125
COURT/ DSS ORDERED CLASSES BELOW:
Domestic Violence Course Intake if doing group: $100
Domestic Violence Course Follow-ups if doing group: $50
Domestic Violence Course Intake if doing individual: $150
